August 22, anytime between 9am-2pm…SCHEDULE/CLASS LIST PICK UP DAY for Secondary and Elementary.
Where? Secondary in the Activity Center & Elementary in the foyer outside the Activity Center, at the triple door entrance.
What do you need to do? Before you come, there are a few things you need to have prepared. The required forms listed below were mailed two weeks ago (if you did not receive the mailing, please contact the office ASAP – these are required before the Schedules/Class List can be released). Remember, Schedules/Class Lists can be released only when this information is submitted :
SECONDARY & ELEMENTARY:
1- On-line Data Verification must be completed. This was from an email sent in early August. If you have not received this email or have questions, please contact Leanne Windle ASAP at 734-459-3505, ext. 3190.
2- Parent Volunteer Form – Only required if you have not already submitted this form for the 2014-15 school year.
3- Parent Promise to Participate – This helps match YOUR skills/desires with PCA’s volunteer team.
4- Once a Year Permission Form, front & back completed, NOTARIZED (Notary will be available), COPY of INSURANCE CARD
5- Medication Form – SIGNED by a PHYSICIAN… ONLY IF, on the Once-a-Year Permission Form, you grant our staff permission to give your student ANY medication (over-the-counter or prescription), it is a legal requirement that this form be on file, signed by a PHYSICIAN.
6- Picture Order Form – Please complete and include payment. SENIORS: You will need to have your picture taken for your I.D., but do not need to complete an Order form.
IF YOU ARE UNABLE TO ATTEND FRIDAY, you can bring the forms (and have the online Data Verification done online) into the office the following week, Tues/Wed/Thurs, 8am-4pm, in order to pick up the student’s Schedule/Class List.
The School Store will be open on Friday for your shopping convenience. Be sure to stop in and check out the great selection of gently used uniform clothing for your elementary students at great prices. All proceeds support our Parent Teacher Fellowship and the work they do at PCA for our teachers.
Be sure to bring your dirty car to schedule/class pickup as well. The senior class will be hosting their car wash in the barn parking lot between 9am-2pm. All donations go toward their senior trip fund. The seniors will be traveling to Costa Rica for a week of fun and ministry in the spring of 2015.
NEW THIS YEAR … There will be representatives from various PCA groups available to answer any questions and convey information. This is a great opportunity to learn more about ways to be involved and support PCA. The PCA Athletic Booster Board will be taking orders for Spirit Wear until September 12th.
